This form is to be filled out for cadets who are ride-along eligible within the South Trail Fire Cadet program. Those submitting this form understand the expectations, responsibilities, and risks associated with riding along with South Trail Fire and Rescue District. Those who submit this form and their dependents will not hold South Trail Fire and Rescue District responsible for any injury, death, or mental trauma associated with the ride along program.
